PURPOSE:
To make a recommendation to the Arizona Department of Liquor Licenses & Control regarding a Series #12 Restaurant Liquor License Agent Change and Acquisition of Control application for Azul Agave, 1561 Palo Verde Boulevard South.
BACKGROUND:
Mr. Joel Zuniga has applied for a Series #12 Agent Change and Acquisition of Control of an existing liquor license for the above location. On June 27, 2016, a letter was mailed to Mr. Zuniga regarding the City’s fees and fingerprint requirements for an Agent Change and Acquisition of Control. After several attempts to contact Mr. Zuniga by telephone, he was sent a letter by certified mail, in which the City Clerk’s Office received a return receipt that the letter was delivered and signed for. As of this date, the applicant has failed to make payment and fingerprints with the Lake Havasu City Police Department or the City Clerk’s Office.
Based on the above, staff recommends disapproval of this Agent Change and Acquisition of Control request.
